By booking an appointment with At Home Blood Draws LLC, you acknowledge and agree to the following terms and conditions:

1. Nature of Services

At Home Blood Draws LLC provides mobile phlebotomy and laboratory specimen collection services only.
We do not provide medical advice, medical diagnosis, medical treatment, or interpret laboratory results. All testing, analysis, and medical recommendations are the responsibility of your healthcare provider or ordering laboratory.

2. Requirements for Blood Draw Appointments

To ensure proper collection and processing, you agree to provide:

• A valid laboratory order or test requisition, if required

• Accurate patient information and insurance details (if applicable)

Incomplete or missing information may result in rescheduling or cancellation of your appointment.

3. Sample Handling and Delivery

At Home Blood Draws LLC:

• Collects specimens following professional healthcare standards

• Properly labels and safely transports samples to the designated laboratory

• Cannot guarantee turnaround times or laboratory processing speeds

• Is not responsible for delays caused by courier services, laboratories, weather, or circumstances outside our control

4. Laboratory Errors & Re-Collections

If a specimen must be recollected due to:

• Laboratory rejection

• Insufficient sample

• Clotting

• Improper kit provided

• Wrong lab order

• Or any issue outside our control

A new appointment fee may apply.

5. Patient Responsibilities

By booking, you agree that:

• You are providing accurate, truthful information

• You understand our service is limited to specimen collection

• You will follow all pre-draw instructions (fasting, hydration, medication instructions provided by your provider, etc.)

• You have the legal right to authorize the blood draw for yourself or the individual you are booking for

6. Safety & Access

You agree to provide a safe, clean, and accessible environment for the phlebotomist to perform your collection.
If the environment is unsafe, unsanitary, or violates professional boundaries, the phlebotomist may refuse service, and a fee may still apply.

7. Payment & Cancellations

Payment is due at the time of service unless prior arrangements have been made.
A cancellation or no-show fee may apply if:

• The phlebotomist arrives and you are not present

• You cancel without sufficient notice

• Required paperwork or kits are not provided

8. Limitation of Liability

At Home Blood Draws LLC and its phlebotomists are not liable for:

• Laboratory errors

• Delays in processing

• Adverse reactions such as bruising, fainting, hematoma, or discomfort (all known risks of venipuncture)

• Medical decisions or interpretations made by others based on laboratory results

By booking, you assume all risks normally associated with blood draws.

9. Consent to Treatment

By booking and receiving services, you provide consent for:

• Venipuncture or specimen collection

• Handling, transportation, and delivery of specimens

• Communication regarding your appointment via email, phone, or text
